The Wild Story of the Taum Sauk Dam Failure
Practical Engineering
Taum Sauk? Where the heck is that? Somewhere in southeast Asia? No, it's in Missouri of all places, named for a Piankeshaw chief.
Curious place. I mean, you never see a lake on top of a mountain. The dam was originally built in 1960-62. It failed in 2005, rebuilt and reopened in 2010.

"nobody thought to check", "nobody realized", "overlooked the possibility that".
Sure they did but they tell their boss about something that will cost the company money to prevent something that's never happened they're a troublemaker. Even if the boss agrees, if he tells his boss then he becomes the troublemaker. So it goes up the line to where the money it would cost comes out of their yearly bonus and the Mrs is counting of Paris this year. But they will surely add their "thoughts and prayers" for the families of those killed and maimed when the shit hits the fan as they pass the blame down the line to underlings on the quarterly call from Wall Street.
